Meals on Wheels of Syracuse is non-profit agency with a great long-term reputation providing nutritious home delivered meals to food insecure older adults and adults with disabilities within the city of Syracuse and some portions of Onondaga County. As a small agency, we value our employees, a respectful work environment, and diversity in our workforce.
We are seeking a full-time Case Manager (7 hours per day weekdays only) to provide in-home assessments and referral assistance to government-funded older adult meal recipients. Ability to engage and interact with older adults with diverse backgrounds and environmental living conditions. Knowledge of support services for seniors in our local area. Must efficiently manage and complete extensive online assessment documenting and have good typing skills. Time management skills essential. Must have a reliable vehicle and valid driver’s license. Bilingual in the Spanish language is preferred. Associate’s Degree in Human Services and one year of case management with older adults, or two-year’s of equivalent experience providing case management to older adults required.
